Hospitalist : Improving care for a faster recovery.

Hospitalists work as part of the health care team to ensure patients and families receive the best information and care during an inpatient stay. Hospitalists are in the hospital 24 hours a day so that they can be responsive to patient needs and family concerns. Your hospitalist team will follow up on tests, adjust treatment schedules as needed and maintain communication with your doctor.

In addition to specialized knowledge of acute hospital care, hospitalists are familiar with specialists and departments within the hospital so that they can effectively assist patients with a smooth and speedy recovery process. Research has shown that Hospitalist Programs may improve the efficiency of hospital care, with shorter lengths of stay and better outcomes.

Extended Stay Rehabilitation : Some extra help before returning to life's routines. Sometimes recovery from a stroke, accident or other illness requires extended rehabilitative care. For these cases, LIFE offers a transitional care unit . Patients staying in the unit have access to services including physical therapy, occupational therapy, speech therapy, advanced therapy programs, short-term rehabilitation and "rehab-to-home" programs.

The foundation helps individuals who have recently experienced a hospital stay by allowing them to continue their therapy and/or skilled nursing care in a convenient, well-staffed location.

HOSPICE
Helping patients through their end of life journey.
When a cure is not possible for individuals with a life-limiting illness, inpatient hospice care may be the best option. LIFE provides inpatient care to help patients and their loved ones through the end of life with dignity and hope.
LIFE has two hospice rooms on the medical/surgical floor, these rooms were specifically designed for end-of-life care. Each room has family accommodations, including a kitchenette, comfortable furnishings and a private restroom. These rooms are served by Registered Nurses with hospice training who manage the established plan of care for hospice services, including frequent communication with the patient's doctor and hospice team.

Why inpatient hospice?
Inpatient hospice care benefits patients who have acute care needs and may not be able to travel to their doctor's office any longer. Inpatient services also provide more effective pain control or acute and chronic symptom management. Families also benefit from 24-hour attention and care.
These conditions and diseases may quality for inpatient hospice care:

Cancer, End-stage heart, lung, kidney or liver disease, HIV, Coma, ALS, Dementia, Stroke & Other terminal conditions as certified by the attending physician.

NON-MEDICAL HOME CARE With an increased desire to "age in place " more and more services are being delivered to the home. Non-Medical home services include but not limited to personal care, assistance with ADL (activities of daily living), Meal (preparation/supply), house-keeping and transportation.

Q. Do I have to have health insurance to become a member of 'TGF' ?
No. Anyone can become a member of the Dr Harish foundation aka TGF at anytime. We do not pre-screen or deny patients because of their age, race, religion, national origin, health condition including the presence of any sensory, mental, or physical disability, sexual orientation, education, economic status, employment status or citizenship.
UNIVERSAL HEALTH INSURANCE
Dr Harish Foundation @ TGF pays the insurance premium towards
ACCIDENT INSURANCE (disability inclusive) - towards PRADHAN MANTRI SURAKSHA BHIMA YOJANA to all TGF members @ Rs.12/yr to be eligible for :
Lakhs to surviving spouse on death
Lakhs on loss of vision to both eyes or both limbs
1 Lakh on loss of vision for 1eye or 1 limb
We are pleased to inform you about the opportunity to cover all TGF members under the Voluntary Senior Citizens Policy. All TGF members are eligible for medical insurance coverage under this scheme. This policy is a comprehensive program, which has been specifically customized for TGF members and offers significant value over regular retail programs available in the market.

Q. Is my health information confidential ?
Yes, Dr. Harish Foundation @ TGF is an independent, privately operated medical company that respects and guards your privacy. We will not share any medical records or personal information with anybody without your express consent.

Q. Do I need an appointment to be seen ?
Yes, so that we can provide the best care for all our patients and help you avoid wait. We ask that you schedule an appointment before you visit. If you have an urgent need, we will make every effort to see you as soon as possible.

Q. What if I have an emergency ?

Q. Can I add family members to my membership? Do I need an appointment to be seen ?
Yes. Spouses, children, parents of enrolled children and other family members can be added to or removed from your billing account at any time. Each patient will have his or her individual medical record and may request to have their care provided by a different health care provider if they wish. However, the membership is presently open only to the Sr.citizens .

Q. Is there a long-term contract ?
No. TGF offers services on a month-to-month basis and you can cancel at anytime. However, to re-enroll, you will pay Rs.500 re-enrollment fee.

Q. Is TGF health insurance?
No. We work directly for our patients to provide direct primary care. Your monthly care fee pays for our primary and preventive care services. TGF does not bill any insurance carrier for our services, and monthly care fees are not reimbursable by any health insurance company, and may not be applied to any insurance plan deductible. Your insurance plan may be billed by others for services such as emergency, hospital, specialty care, laboratory tests, diagnostic imaging, prescription drugs or other goods and services that are ordered by your insurance designated health care provider but are not performed or provided in our office.
Our rural chapter pays the insurance premium towards PRIME MINISTERS JEEVAN SURAKSHA YOJANA.

Q. If I am in a managed care healthcare plan currently, may I join TGF too?
Yes. Your monthly fee provides you with TGF primary care regardless of your insurance or managed care plan.

Q. Do you currently offer TGF memberships to union members?
Yes. Several union members are members of TGF through trust plans or collectively bargained agreements.

Q. What if need to see a specialist?
When your medical needs extend beyond the scope of primary care,our empanelled specialist will see you.

Q. How much is the charge and how quickly can I sign up?
TGH charges each patient a low, age-based monthly fee, regardless of insurance status or pre-existing conditions.

Having a Primary care Provider you is important, since you and your ur PCP/PCHP will work together as partners in your healthcare. When you have a health question, or when you want to talk about how to stay well and prevent future health problems, your PHCP is just a call away.

We encourage you to consult your PHCP early and often for day-to-day primary care,urgent care,preventive care,and chronic disease management needs. We believe Through proactive managemernt of any health risks or issues, we can identify and manage conditions early when their easier- and less costly-to treat. And, since we know your time is valuable, we offer a variety of options for your convenience :

Care Philosophy
Each PHCP works with a limited number of patients so your personal time with him is unhurried, will have more time to spend with you, your provider will get to know you, help you reach your health goals, and will also help co-ordinate any necessary specialist or hospital care.

TGF strives to remove all barriers to care, so that when you have a healthcare need or question, you wont have to hesitate or put off getting the attention you need. There are no limits on the number of visits, whether they are by phone, in person or over secure Email. We encourage you to reach out to us whenever you have a health concern so that we can answer your question promptly and help you decide on the best plan of care. Our goal is to keep you healthy and prevent unnecessary complications from developing.

You have the right to choose your personal health care provider and to change this selection at any time, for any reason. All reasonable efforts will be made to accommodate any request, but only if your new choices patient panel is open to new patients.
You have the right to receive accurate and easily understood information about Dr. harish foundation health care services, health care professionals and health care facilities.
Dr. Harish Foundation will make its best effort to provide assistance so you can make informed health care decisions.
You have the right to considerate, respectful, and non-discriminatory care from your health care provider(s).
You understand that you are responsible for communicating clearly and respectfully with any provider.
You have the right to know all of your treatment options and to participate in your health care decisions. Parents, guardians, family members, or other individuals whom you designate may represent you if you cannot make your own decisions.
You have the right to speak in confidence with your PHCP and to have your your health care information protected.
You also have the right to review and receive a copy of your personal medical record and may request that your health care provider(s) amend your record if you feel it is inaccurate or incomplete.
You have the right to a fair, fast and objective review of any complaint you have against your health care provider(s) or any other staff, including complaints about wait times, operating hours, conduct of personnel, business practices, and adequacy of or our services, you agree to notify us immediately so your concerns may be addressed in a timely manner.
