LIFE Programs : Adressing the health care needs of the senior citizen…
It is often difficult to meet the needs of the elderly while they live independently in their own home.
LIFE is a Program of All-Inclusive Care for the Elderly (PACE). This model of care is centered around the belief that it is better for older adults and for their families if their chronic care needs are provided in the community whenever possible. This is an innovative program in that it provides services to those who would otherwise need nursing home care while they remain in the community in their own home or the home of loved ones.
The LIFE Program promotes independence and the highest levels of functioning while allowing choice and dignity for our members and their families.
LIFE provides all needed preventive, primary, acute, and long term health care services so that qualified (or eligible), older individuals continue to live in their homes as long as possible. We believe that older adults with chronic care needs often do better when they continue to live in the community.

Many of these services are provided at the LIFE adult day center. Members receive full medical and preventive care, and may participate in the range of activities offered each day.
Care is provided by a team of licensed professionals who work to meet the individual needs of each member. The team remains involved in the member's care even if they should need to be hospitalized or if they require short or long term treatment in a nursing home. The care team is made up of primary care physicians, nurse practitioners, registered nurses, social workers, therapists, personal care workers, and other dedicated staff.

The advent of nuclear families has given rise t o a piquant situation. The elderlys are generally ignored-treated as pariahs- and even worse for those who don't contribute anything to the family income. The senior citizens are frustrated, go into depression and long for the company of their children, grand children or just anybody with whom they could interact during their golden years.
The DAY-CARE CENTRE FOR THE ELDERLY, called "LIFE CENTRES" managed by the Dr HARISH FOUNDATION provides oppurtunitys for Companionship, Recreation, health care and Nutrition and even shelter during emergencies.

LIFE CENTERs would be open on all days,24/7,365 days. Older people generally interest themselves with religion & spiritually. The foundation would organize lectures and visits to religious places and encourage participation in cultural events.
All festivals would be celebrated together at the centre.
The Mobile Medicare van providing pri-health care services will be manned by Medical Professionals and Health Assistants. Members requiring assisted care for minor ailments will be admitted to the attatched LIFE CENTER MEDICAL HOME. Those requiring investigations or specialized treatment would be treated at the Secondary care hospital or brought to the main centres at bengaluru.
Seminar Room
• A seminar room that seats up to 100 attendants, equipped with the latest audio visual technology is also available on this floor. It will be utilised for training, workshops, lectures and courses on a wide range of health and lifestyle topics.
PACE Programme:
PROGRAMME OF ASSISTED CARE FOR ELDERLY care improves the health & life of frail senior citizens with complex needs. Working together a team of doctors, nurses & social workers use geriatric knowledge & technology to improve the patient care – not just in the clinic but in the patients home & community.

OBJECTIVE OF PACE PROGRAMME
THE AIM OF THE PACE Program is to keep Members of the GOLDEN FAMILY as healthy s possible ,if left without care are likely to require extensive acute and expensive hospital care.

"Old age homes" are enrolled as "PACE AFFILIATES" & the residents are treated on par with The Golden Family (TGF) Members.
Senior Citizens ' fora viz "DIGNITY FOUNDATION",NIGHTINGALE ELDER CARE and other such institutions are enrolled as "TGF ASSOCIATES" and their members treated on par with TGF members.
Under its PACE programme the Dr Harish foundation adopts all old-age homes as its "PACE AFFILIATE" and all residents are treated on par with The Golden Family members for a nominal fee of just Rs.10/- per day.

A Geriatric Care Manager can help families who are caring for older relatives.
Geriatric Care Management plans and coordinates the care of the elderly and/or disabled to improve their quality of life and to maintain their independence. The Geriatric Care Manager is trained in any of several fields nursing, gerontology, social work, or psychology, with a specialized focus on issues related to aging and elder care. Just as there are patient advocates, think of a geriatric care manager as an advocate for seniors. The Geriatric Care Manager is an experienced guide and resource for families.
What Does a Geriatric Care Manager Do?
Older parents are not always in the same geographic location as their children. Geriatric Care Managers serve as stand in providers when families cannot be around.
The duties of a care manager include:
Conducting care-planning assessments to identify needs.
Putting a care plan together and then executing that plan.
Screening, arranging, and monitoring in-home help.
Acting as a liaison to families at a distance.
Assisting with moving their clients to or from different care settings.
Reviewing financial, legal, or medical issues and referring clients to experts for same such as estate planning, living wills, and family trust experts.
Providing crisis intervention.
Providing client and family education.
Visiting clients on a regular, routine basis to make sure they are safe, doing well, eating properly, and taking needed medications.
Making necessary medical appointments and assuring client gets to them.
Identifying agencies and/or social services and other programs that client can avail.
Monitoring the elder's finances and paying bills.
GCMs offer something else for the client-the peace of mind that comes with knowing a loved one is being taken care of properly.
The goal is to help people stay as independent as possible while offering necessary help such as meals, personal care and medication management ,providing lifestyle with life care.
